La consultation de gynécologie touche à l'intimité. Le vécu de ces consultations par la patiente conditionnerait son suivi. Le bon déroulement de la consultation gynécologique est donc un enjeu de santé publique. De nos jours, certaines pratiques professionnelles ont été critiquées et qualifiées de "violences gynécologiques". Elles sont dénoncées par les patientes, les médias et les autorités de santé. On constate un manque de sensibilisation sur le sujet lors de la formation des professionnels de santé de la femme, souvent centrée sur la technique. Nous avons donc créé une fiche à l'aide de la méthode Delphi afin de guider les professionnels de santé vers une pratique bienveillante. Pour cela, nous avons recruté plusieurs experts possédant des compétences dans le domaine de santé de la femme et ayant un intérêt pour une pratique bienveillante. Chaque expert a pu donner son avis sur des propositions rédigées à la suite de notre recherche bibliographique. Un consensus sur le contenu de cette fiche a été obtenu à l'issue de trois tours. Cette fiche se compose de 73 items, répartis en cinq catégories : la communication, l'examen physique avec l'examen pelvien, le toucher vaginal et la palpation mammaire, le premier examen gynécologique, le dépistage des cancers gynécologique et la contraception. La méthode Delphi a permis une réflexion sur le rôle du soignant, sur les freins à la réalisation d'une consultation gynécologique bienveillante et la difficulté à remettre en question ses pratiques. Une formation adaptée et continue est à mettre en place afin d'éviter des pratiques de violences gynécologiques. Ce travail n'a pas la prétention de dicter une ligne de conduite unique. C'est avant tout une réflexion sur nos pratiques actuelles et des pistes pour améliorer la prise en charge de l'ensemble des patientes.

Introduction : L'information médicale sous forme de fiche d'information patient (FIP) est essentielle dans la relation medecin-malade. Elle est un souhait des patients et un support pour le médecin. En matière de suivi gynécologique, l'adhérence et la participation des femmes aux dépistages sont insuffisantes du fait souvent d'un manque de connaissances. Il paraissait donc nécessaire de réaliser une FIP dans ce domaine. Objectif : Concevoir une FIP sur le suivi gynécologique en suivant une méthodologie valide scientifiquement puis l'évaluer auprès des femmes afin d'obtenir un document final adapté, compréhensible et clair, diffusable aux médecins. Méthodes : Mise en place d'un cahier des charges avec une liste d'étapes clés à suivre pour concevoir cette FIP. Puis, réalisation d'une enquête en face-à-face auprès de 12 femmes, afin d'évaluer la compréhension et la présentation du document, recueillir leurs opinions et les propositions de modifications. Résultats : La brochure intitulée « le suivi gynécologique, pour une meilleure santé de la femme» a ainsi été conçue puis validée par des experts. Sa lisibilité statistique était bonne. L'enquête montrait que la brochure permettait une bonne localisation des informations, était bien comprise et appréciée des femmes, leur apportant des informations nouvelles. La FIP a ensuite été finalisée en tenant compte des remarques des patientes puis diffusée par mail aux médecins généralistes de Haute Savoie. Conclusion : Cette brochure ayant pour but de sensibiliser les femmes à leur suivi gynécologique constitue un rappel écrit adapté et utile qu'il serait interressant de proposer aux femmes afin qu'elles puissent s'y référer si besoin.
