Avec le nouveau plan cancer (2014-2019), le renforcement de la coordination des soins au-delà de l’hôpital apparaît comme un objectif prioritaire dans l’amélioration de la prise en charge globale du patient souffrant d’un cancer. L’objectif de ce travail est de proposer un outil qui non seulement apporte des informations au patient afin qu'il soit acteur de sa propre prise en charge mais aussi aux professionnels de santé de ville, véritable relais et rempart de sécurité pour le bon usage des médicaments et gestion des interactions médicamenteuses et des effets indésirables (médecin traitant et pharmacien d'officine). Le dispositif permet d'améliorer la qualité de prise en charge initiale des patients intra-muros puis de garantir l’échange d’informations sur la chimiothérapie intraveineuse entre l’hôpital et la ville précieuse aux bon conseils. Matériels et Méthodes : Dans un premier temps, un modèle standardisé de document d’information a été conçu à partir de l’analyse de la littérature. Ce document a été réalisé pour chaque protocole de chimiothérapie injectable utilisé dans le cancer du sein. Ensuite, une consultation pharmaceutique fut mise en place autour de 4 axes: Conciliation, analyse d’interaction, information du patient (entretien pharmaceutique), informations des acteurs de proximité. Résultats : Les plaquettes d'informations synthétisent les informations générales concernant le protocole de chimiothérapie, les effets indésirables fréquents liés au protocole de chimiothérapie, les points clés de leur prise en charge ambulatoire (traitements médicamenteux, conseils hygiéno-diététiques), les interactions médicamenteuses (allopathie, phytothérapie et compléments alimentaires) avec les molécules d'anticancéreux et les principaux éléments de surveillance. La consultation pharmaceutique permet de faire une analyse pharmaceutique de la totalité des traitements des patients (avec et sans ordonnances) et permet de les sensibiliser aux interactions médicamenteuses et/ou alimentaires. Conclusion : Le médecin traitant et le pharmacien d’officine occupent une place importante et privilégiée dans le système de santé français de par leur disponibilité et leur proximité avec le patient atteint d’un cancer. Ce projet propose des outils d’informations spécifiques, originaux et pertinents qui visent à répondre aux attentes des patients et des professionnels de santé : sécuriser les soins en prévenant les risques iatrogènes, assurer la continuité des soins entre hôpital et ville afin de ne pas rompre l'accompagnement du patient, et mettre le patient au centre de sa prise en charge.
