Importance : À ce jour aucun traitement n'a prouvé son efficacité contre le virus SARS-CoV-2 responsable de la pandémie de la COVID-19. Objectif : Comparer et classer les traitements contre le SARS-CoV-2 en termes d'efficacité et innocuité. Source des données : La recherche bibliographique a été faite à partir des moteurs de recherche MEDLINE et librairie Cochrane (CENTRALE) jusqu'au 5 septembre 2020, en utilisant les mots clés suivants : 'COVID-19'ou 'SARS-CoV-2' et 'traitement' Études sélectionnées : ont été inclus, tous les essais cliniques randomisés sur le traitement contre le SARS-CoV- 2. Ont été exclus les essais cliniques non randomisés ainsi que ceux dont les informations sur les critères de jugement étaient manquantes. Extraction des données et synthèse : nous avons utilisé l'outil Cochrane pour évaluer le risque de biais. Le nombre de patients décédés 28 jours après la randomisation et le nombre d'évènements indésirables rapporté ont été extraits. Une méta-analyse en réseau basé sur un modèle à effets aléatoires a été conduite. Le protocole d'étude est enregistré auprès de PROSPERO, numéro CRD42020176977. Principaux résultats et mesures : Le risque relatif (RR) et l'IC à 95 % ont été calculés pour la mortalité toutes causes confondues à 28 jours, l'amélioration clinique, tout événement indésirable et la charge virale. Résultat : Au total 25 essais randomisés comparant 17 traitements différents chez 11597 patients hospitalisés pour la COVID-19, ont été inclus. L'âge moyen de participants varie entre 29,7 et 70 ans, la majorité d'entre eux étaient des hommes [7259(62,6%)]. La qualité globale de risque des biais était modérée. 28 jours après randomisation, 2228 sont décédés. Le remdesivir pendant 10 jours (RR 0.69, 95% CI [0.48 - 0.99]) comparé aux soins standards, et l'hydroxychloroquine à faible dose (0.38, [0.17 - 0.89]) comparé à une dose plus élevée étaient associés à une réduction de la mortalité toutes causes confondues à 28 jours. 2766 patients ont eu une amélioration clinique, avec un risque relatif de 1.21 (1.00 - 1.47) pour le remdesivir pendant 5j par rapport aux soins standards. 1889 événements indésirables dont 598 sévères ont été déclarés. Comparés aux soins standard, le remdesivir pendant 5 et 10 jours, le lopinavir / ritonavir et la dexaméthasone réduisaient le risque de tout événement indésirable sévère de 52 % (0,48, 0,34 - 0,67), 24 % (0,77, 0,63 - 0,92), 40 % (0,60, 0,37 - 0,98) et 50 % (0,50, 0,25 - 0,98) respectivement. Conclusion : Dans cette méta-analyse en réseau, parmi les patients hospitalisés pour la COVID-19, le traitement par remdesivir pendant 10 jour, est associé à une réduction de la mortalité à 28 jours, et à un taux d'amélioration clinique plus élevé mais aux événements indésirables sévères.
