Introduction. La couverture vaccinale vis-à-vis de la rougeole stagne et est insuffisante en France et dans le monde, ce qui a permis sa réémergence actuellement observée. L'essor d'Internet a engendré une plus grande diffusion d'idées anti-vaccination et majoré l'hésitation vaccinale. Mais les nouvelles technologies pourraient aussi permettre le développement d'outils d'information et d'aide-mémoire sur la vaccination. Méthodes. Une étude épidémiologique évaluative longitudinale, non randomisée, prospective, avait pour objectif d'évaluer l'impact d'une campagne d'information par courriel quant à l'amélioration de la couverture vaccinale ROR dans la Loire. Un courriel a été envoyé par la CPAM de la Loire entre octobre 2015 et septembre 2016 aux parents d'enfants de 13 mois vivant dans le département (groupe action). Six mois plus tard, les données ont été récupérées auprès des pharmacies pour comptabiliser l'achat des doses vaccinales du groupe ayant reçu le courriel et celui du groupe contrôle (n'ayant pas reçu de courriel). Résultats. L'étude a inclus tous les enfants du département de cette classe d'âge, soit 9 233 enfants. Le taux de vaccination dans le groupe intervention était de 86.6 % alors qu'il n'était que de 83.5 % dans le groupe témoin, soit une majoration de 3.1 %. Conclusion. L'envoi d'un courriel aide-mémoire aux parents d'enfants en âge d'être vaccinés par le ROR est associé à une meilleure couverture vaccinale. Cet effet pourrait être amélioré en le combinant à un outil numérique d'aide à la décision.

The 2018 FAO-OIE-WHO (Tripartite) zoonoses guide, “Taking A Multisectoral, One Health Approach: A Tripartite Guide to Addressing Zoonotic Diseases in Countries” (2018 TZG) is being jointly developed to provide member countries with practical guidance on OH approaches to build national mechanisms for multisectoral coordination, communication, and collaboration to address zoonotic disease threats at the animal-human-environment interface. The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

With decentralization and urbanization, the debts of state and local governments and of quasi-public agencies have grown in importance. Rapid urbanization in developing countries requires large-scale infrastructure financing to help absorb influxes of rural populations. Borrowing enables state and local governments to capture the benefits of major capital investments immediately and to finance infrastructure more equitably across multiple generations of service users. With debt comes the risk of insolvency. Subnational debt crises have reoccurred in both developed and developing countries. Restructuring debt and ensuring its sustainability confront moral hazard and fiscal incentives in a multilevel government system; individual subnational governments might free-ride common resources, and public officials at all levels might shift the cost of excessive borrowing to future generations. This book brings together the reform experiences of emerging economies and developed countries. Written by leading practitioners and experts in public finance in the context of multilevel government systems, the book examines the interaction of markets, regulators, subnational borrowers, creditors, national governments, taxpayers, ex-ante rules, and ex-post insolvency systems in the quest for subnational fiscal discipline.
