Introduction : La littérature est pauvre concernant l'écologie des infections intra-abdominales (IIA) en pédiatrie. De plus, les recommandations préconisent dans ces infections d'adapter les protocoles d'antibiothérapie probabiliste à l'écologie microbienne locale. L'objectif principal de l'étude était de faire un état des lieux des germes retrouvés et de leur profil de résistance dans les IIA pédiatriques au CHU de Rennes. L'objectif secondaire était l'analyse des facteurs de risque microbiologiques de complication post-opératoire. Matériel et méthode : Il s'agit d'une étude monocentrique observationnelle prospective se déroulant de novembre 2017 à janvier 2019. 145 enfants atteints d'IIA ayant bénéficié de prélèvements microbiologiques ont été inclus. Résultats : Sur les 145 enfants, l'IIA était d'origine appendiculaire dans 96.6%. 69 prélèvements (47.2%) étaient positifs. 81,2% des prélèvements étaient polymicrobiens. Les germes retrouvés sont majoritairement des entérobactéries (75,4% de prélèvements à E. coli et 10,1% à P. aeruginosa), des streptocoques (37.7%), des anaérobies (59,4%, 18,8% à B. fragilis), et de rares entérocoques. La sensibilité à l'amoxicilline-clavulanate était de 78,3%, à la pipéracilline-tazobactam de 97,1% et à l'association d'une céphalosporine de 3eme génération (C3G) et de métronidazole dans 89,9%. On n'observait aucune bactérie porteuse de béta-lactamase à spectre élargi. Seul le caractère polymicrobien est identifié comme un facteur de risque de complication post-opératoire. Conclusion : Au vu des résultats observés, et sachant que la pathogénicité du P. aeruginosa reste incertaine, une antibiothérapie probabiliste par C3G - métronidazole semble être une bonne alternative.
