Entre 2007 et 2009, le dosage de la vit D a augmenté de 250%. Face à cet engouement majeur, la HAS a émis un rapport sur l'utilité du dosage de la vitamine D et un déremboursement partiel s'en est suivi en 2014. La vitamine D est une hormone synthétisée au niveau cutané grâce aux UVB ; ses effets sur le métabolisme osseux sont connus depuis de nombreuses années mais on lui présume un rôle bien plus large du fait de la présence de ses récepteurs dans un très grand nombre de tissus. Notre étude cherchait à comprendre ce phénomène, à mettre en évidence les éléments motivationnels des prescriptions du dosage de la vitamine D par les médecins généralistes. Il s'agit d'une étude qualitative avec entretiens semi-dirigés de 11 médecins généralistes des Pyrénées Orientales. On a observé un véritable effet de mode, avec à son origine un grand nombre de publications dans des revues médicales sur les potentiels effets extra osseux de la vitamine D (maladies auto immunes, cancer du colon, ... ). Comme cela a déjà été démontré dans d'autres études, les visiteurs médicaux ont aussi influencé les prescripteurs. Les généralistes ont suivi les pratiques de confrères spécialistes et ont été surpris par les résultats très souvent bas des dosages. Les valeurs seuils sont très souvent critiquées, trouvées trop élevées ou désuètes. Depuis le déremboursement partiel de la vitamine D, tous les médecins ont changé leurs habitudes de prescription en ne le prescrivant plus que très rarement. Avec ces recommandations économiques, il semblerait que le nombre de prescription aille en nette décroissance. Il subsiste encore de nombreuses questions concernant les effets extra osseux de la vitamine D, des études contrôlées sont attendues pour montrer un lien de causalité et une valeur seuil associée à ses effets
