Excerpt from The Thyroid Gland in Health and Disease A period of sick leave from active service has afforded me an opportunity to incorporate into one volume the results of some fourteen years of study of the thyroid apparatus and its aetiology. When, in 1902, I undertook the investigation of the aetiology of its maladies a maze of hypotheses and uncertainties surrounded the subject. Knowing nothing, premising nothing, I determined to examine it from the combined epidemiological, clinical, experimental and pathological points of view. Some years of extensive epidemiological research cleared the ground of many false hypotheses, resolved some uncertainties, and brought to light certain facts which suggested an infectious or toxic origin for some of the thyroid's most common disorders. Clinical experience during the same period added further facts which strengthened this supposition; observations on man and experimental work on a large scale on animals converted the supposition into what I believe to be fact, and pathological studies have since confirmed my belief. Meanwhile it became necessary to revert to the study of the normal thyroid gland in order fully to understand the various processes of abnormality. As a result of this study a number of important points relating to the glands normal range of physiological action were elucidated, whereby it became possible to interpret the pathological changes to which various experimental procedures had given rise. The epidemiological work received confirmation from the observations made on artificially-bred trout in America, by Marine and Lenhart and by Gaylord; the clinical and therapeutic facts were supported by the researches of Messerli; the earlier experimental findings were confirmed by Sazaki, working under Wilms of Heidelberg, by Gaylord in America, and by Messerli in Switzerland. Excerpt from Graves' Disease With and Without Exophthalmic Goitre The object of this treatise is to emphasize the fact that the constitutional and general derangements which are characteristic of Graves' Disease, constitute the disease, and not the condition of the thyroid gland, or of its accessories. To what extent a spe cific disorder of the thyroid is essential to the origin and contin ued progress of Graves' Disease, is rendered doubtful by the oc currence of cases of Graves' Disease in its severest and even fatal forms who show, however, no evidence of the thyroid being in volved at all. In this treatise the clinical histories of twenty-eight patients who at no time showed any signs of Ex0phthalmic Goitre are compared, symptom by symptom, with the clinical histories of forty-two patients who did have Graves' goitre, each of both classes occurring in my own private practice. The comparison is made to demonstrate that this disease may have no recognizable, and therefore probably no necessary, relation to any state of the thyroid gland. The details of these histories may make tedious reading, but in no other way can this important clinical fact be established, involving as it then does very difierent conclusions about the pathology and treatment of Graves' Disease from the views of those who define Basedow's Disease as an infection of the body through morbid activity of the thyroid gland.
