Résumé :Que sont devenus les internes de médecine générale de l'Océan Indien issus des promotions 2004 à 2012 ? Dans un contexte global de baisse des effectifs des médecins généralistes et d'une densité médicale critique dans certains territoires, nous avons étudié le devenir des internes de médecines générale. L'objectif principal était de connaitre la proportion d'anciens internes installés en libéral. Nous avons réalisé une étude observationnelle descriptive transversale par questionnaire électronique portant sur leur situation personnelle et professionnelle en 2017. Nous avons collecté 223 réponses, soit un taux de participation de 86,8%. Les anciens internes étaient âgés en moyenne de 32,6 ans et 57,8% étaient des femmes. 36% étaient salariés, 30% exerçaient dans leur cabinet ou en collaboration et 27% étaient remplaçants. 57,9% des installés étaient toujours dans l'Océan Indien (OI) en 2017. Ils exerçaient majoritairement en cabinet de groupe. Les remplaçants toujours présents à la Réunion envisageaient de s'y installer dans 62% des cas. Parmi eux, on retrouvait 92% des médecins originaires de l'OI. A terme, il resterait 37% des anciens internes dans l'OI. Leurs motivations principales étaient le cadre de vie et une relation médecin/patient appréciée. La subdivision Océan Indien (OI) a été créée suite à la réforme des Epreuves Nationales Classantes en 2004. L'objectif principal de notre étude était de dresser le profil professionnel des internes de médecine générale de l'OI des promotions de 2004 à 2008. L'étude a été réalisée à partir d'un questionnaire électronique adressé, entre juin et août 2012, à l'ensemble des 98 internes. Seuls 3 n'ont pu être contactés. Le taux de participation était supérieur à 90% (n=87). 55,2% étaient des femmes. L'âge moyen était de 30,2 ans. Environ 80% avaient choisi la médecine générale par conviction. Ils étaient globalement satisfaits de leur internat avec une note moyenne de 8/10. 72,4% étaient domiciliés dans l'OI au moment de notre étude. Le remplacement constituait le principal mode d'exercice post-internat (67,8%). 1/4 étaient salariés de l'hôpital dont la moitié dans les services d'urgences. Seulement 8% des généralistes étaient installés (6 installés, 1 collaborateur et 2 collaborateurs avec une activité mixte) avec un délai moyen d'installation de 1,4 an après la fin des études. 63,4% des remplaçants avaient un désir d'installation dans l'OI d'ici à 5 ans, dont 20% avec un projet en cours. Le cadre de vie et la découverte d'une nouvelle région constituaient les principales motivations du choix de leurs venues et l'envie de rester. Le rapporchement familial était la cause principale de départ suivi de raisons géographiques (éloignement et insularité). La création récente en 2012 d'un CHU à la Réunion et d'un département de Médecine Générale va probablement modifier les modalités d'exercice. Il serait intéressant de réaliser une nouvelle étude dans cinq ans sur cette même population, encore jeune, pour vérifier si les projections d'installation se sont confirmées et si le statut de remplaçant est le fait d'un simple démarrage de carrière ou s'il se prolonge.
