Introduction : la médecine générale actuelle repose sur l'Evidence Based Medicine (EBM) dont un des piliers fondamentaux est : les recommandations de bonne pratique clinique. Ces dernières ne sont pas toujours simples à appliquer et à utiliser par le médecin généraliste. Ce dernier doit faire face à des demandes de conseils sur l'usage des nouveaux outils numériques de santé (objets connectés, application mobile de santé). Les outils de la e- santé tels que les systèmes d'aide à la décision pourraient être une réponse à ces problématiques. Le projets ApiApps vise à élaborer un système d'aide à la prescription d'applications mobiles en santé adaptées au profil du patient. Notre travail s'inclut dans ce projet et avait pour objectif de traduire les recommandations de santé de dix pathologies prévalentes en médecine générale d'un format textuel à un format informatique pour un usage en pratique courante dans les logiciels métiers. Et comme objectif secondaire une description de ces règles informatiques. Méthode : dix pathologies ont été sélectionnées à partir des études de prévalence en médecine générale. Onze recommandations francophones ont été analysées. Les règles ont été extraites des recommandations par deux experts indépendants. Une cible (patient ou médecin), une thématique (sélectionnée parmi les Q-Codes), un niveau de certitude et un niveau de preuve ont été attribués à chaque règle. Les discordances entre les deux experts ont été résolues par consensus. La pertinence des règles dans le cadre du projet ApiApps a été déterminée en fin d'analyse. Résultats : 656 règles ont été établies, 57% avec une cible « médecin » et 43% « patients ». Les règles sont principalement classées dans la catégorie « comportement de prescription » à 36 % suivi de la « « catégorie connaissance du patient » et « compétence clinique » 21 % chacune. 70 ont été sélectionnées comme pertinentes pour le projet ApiAppS, telles que la règle : Si « diabète de type 2 », ALORS « suivi des glycémies » qui permettrait d'enclencher une proposition de prescription d'une application de suivi glycémique. Les thématiques les plus fréquentes de ces règles ApiApps étaient l'auto-soin (64%), l'éducation à la santé (23%) et les connaissances du patient (17%). Conclusion : ce travail illustre la nécessité que les prochaines recommandations comprennent également leur transcription sous forme de règle informatique directement utilisable dans les systèmes d'information et d'aide à la décision. Il soulève également la question de l'élaboration de recommandations à destination du patient et de la place de son savoir.
