Objectifs : Observer le mode d'accouchement des grossesses gemellaires, voie basse ou cesarienne, en fonction du type de gemellite, de la presentation et du poids des jumeaux, de l'age gestationnel, des antecedents obstetricaux et sa repercussion sur la morbidite neonatale. Materiels et methodes : Nous analysons le mode d'accouchement des grossesses gemellaires, entre le 1er janvier 1994 et le 31 decembre 1999, dans le service de gynecologie obstetrique du CHU de St-Etienne. L'age gestationnel au moment de l'accouchement n'est pas un critère d'exclusion. L'etude porte sur 249 patientes et 498 enfants, après avoir exclu un cas d'interruption therapeutique de grossesse. Trois cas de mort fœtale in utero, un pour le 1er jumeau (J1) et 2 pour le 2eme jumeaux (J2) seront secondairement exclus pour permettre l'étude de deux criteres de morbidite fœtale : Apgar

L'évaluation de la voie d'accouchement la moins risquée en cas de grossesse gémellaire avec un siège premier est complexe en raison de l'hétérogénéité des pratiques de prise en charge. Cette situation est soumise à de nombreuses controverses puisqu'elle représente un facteur de risque de complications supplémentaires à ceux des grossesses gémellaires lors de l'accouchement. Alors que le Collège des gynécologues obstétriciens français conclut que les données actuelles sur le sujet ne permettent pas de recommander une voie d'accouchement plutôt qu'une autre, les études anglo-saxonnes prônent la césarienne prophylactique. Nous avons réalisé une étude rétrospective sur une période de six ans à la maternité du CHU d'Amiens-Picardie afin d'y analyser les pratiques concernant l'accouchement des grossesses gémellaires avec le premier jumeau (J1) en siège pour y comparer le devenir maternel et néonatal. Nous avons pu constater que la tentative voie basse était envisageable lorsque celle-ci était acceptée par la patiente et après évaluation des critères de sécurité de cet accouchement. Il n'y avait pas d'excès de risque néonatal et maternel. La parité n'avait pas d'influence sur le mode d'accouchement. Les facteurs prédictifs d'échec de la voie basse retrouvés, ont été la présentation céphalique du deuxième jumeau ainsi que le poids de naissance de J1. Néanmoins, il est nécessaire de respecter les indications de césarienne programmée. Nous devons insister auprès des professionnels sur l'importance de l'enseignement aux étudiants de ce type d'accouchement, afin de les expérimenter à ces situations qui sont certes rares, mais qui peuvent rapidement être source de complications.
