Objectif. - La voie d'accouchement d'un foetus en présentation du siège est un sujet de débat depuis les années 1960 et plus encore depuis l'essai randomisé de 2000. Bien qu'existent en France des recommandations concernant les critères d'acceptabilité de la voie basse des présentations podaliques, on remarque une grande disparité de pratiques entre les maternités françaises. Dans notre centre, la décision de voie d'accouchement est laissée à l'appréciation du médecin responsable du suivi de la grossesse. L'objectif de cette étude a été d'évaluer les pratiques obstétricales pour l'accouchement du siège à terme au CHU Grenoble Alpes et de proposer, le cas échéant, des pistes d'amélioration. Matériel et méthodes. - Étude rétrospective et analytique sur dossiers à l'aide d'une grille de recueil de données, d'une population de 163 patientes ayant accouché à terme d'un foetus en présentation du siège à la maternité du CHU de Grenoble Alpes entre le 01/01/2017 et le 31/01/2019. Résultats. - Cent cinquante-quatre patientes ont été inclues sur la période étudiée (9 cas de non inclusion). Pour 51 % des patientes une césarienne a été programmée (une seule patiente a accouché voie basse parmi elles) et dans 23 % des cas, l'accouchement s'est déroulé par voie vaginale. Parmi les 45 % des patientes considérées comme éligibles à une tentative de voie basse, seules 44 % de celles-ci ont finalement accouché par voie vaginale (66 % d'échec de tentative de voie basse). Il faut noter que notre série comporte 5 % de siège inopiné. La morbidité sévère néonatale a concerné 2 nouveau-nés du groupe “voie basse acceptée”, dont un est né par voie basse et l'autre par césarienne. Le bassin considéré comme transversalement rétréci et le refus maternel d'accoucher par voie vaginale, représentaient respectivement 60 % et 30 % des césariennes programmées. Les patientes avec utérus cicatriciel n'étaient pas contre-indiquées à l'accouchement par voie basse pour cet unique motif, mais seules 12 % d'entre elles ont accouché par voie vaginale. Discussion et conclusion. - Une formation optimale théorique et pratique des jeunes obstétriciens, la mise en place d'un protocole de service, et la discussion collégiale de ces dossiers, nous semblent être des pistes d'amélioration de nos pratiques dans l'accouchement du siège singleton à terme. Une fiche d'information à l'intention des patientes pourrait nous aider à promouvoir l'accouchement par voie vaginale.
