L'allongement de l'espérance de vie s'accompagne de fait, d'un vieillissement global de la population française. Le vieillissement se manifeste par des changements physiologiques importants ayant pour conséquence l'apparition de multiples pathologies chez le sujet âgé. Ces polypathologies vont souvent nécessiter la mise ne place de traitements médicamenteux. Cette polymédication expose le sujet âgé au risque iatrogénique médicamenteux. Celui-ci est fréquent, peut-être grave d'emblée ou de révéler par une symptomatologie atypique. Les problèmes iatrogéniques sont à l'origine de 10% des hospitalisations chez les sujets de plus de 65 ans. Cette iatrogénie médicamenteuse représente un coût économique et humain très important dans la population âgée gériatrique. Donc, la prévention de l'iatrogénie médicamenteuse chez le sujet âgé devient un enjeu majeur de santé publique. Au sein d'une unité de soin hospitalière, les professionnels de santé peuvent facilement échanger des informations mais qu'en-est-il en ville ? Le pharmacien d'officine et le médecin traitant exercent souvent de façon isolée et n'obtiennent pas forcément des informations de retours d'hospitalisations. Afin d'améliorer et donc de garantir une continuité des soins après l'hospitalisation, nous avons pensé à la création d'une Fiche de Liaison Pharmaceutique (FLP) qui serait remise au pharmacien « correspondant » désigné par le patient ainsi qu'au médecin traitant. Cette FLP a donc été l'objet de ce travail avec sa création, sa formulation et son test au sein du service de médecine aigue et de soin de suite et réadaptation du pavillon Chatin au CHU de Grenoble.
