In recent years, the prostate cancer diagnosis rate has increased dramatically, with a slower increase in age-specific mortality. At least in part, the increasing incidence undoubtedly reflects more aggressive efforts at early detection of prostate cancer, particularly through the use of a new blood test, prostate-specific antigen (PSA). This background paper examines the implications of a potential Medicare benefit to cover prostate cancer screening using a combination of the PSA and digital rectal examination (DRE), a time-honored test performed in the physician's office. OTA concludes that the evidence of benefit for prostate cancer screening is substantially weaker than for preventive services already covered by Medicare. Because the scientific knowledge is limited, but the consequences of prostate cancer and its treatment are serious, an informed patient and his physician could equally well decide to receive screening or forgo it. Medicare reimbursement could be one means of insuring that out-of-pocket expenses not impede well-informed decision-making between physician and patient about whether to receive screening.

Prostate cancer is a common form of cancer in men. In fact, prostate cancer has the highest incidence of any cancer in men. Prostate cancer is not only common, it carries a significant financial burden. The cost of prostate cancer is 11.3% of all cancer expenditures. If after a DRE the prostate is enlarged, or if the PSA is elevated, then a biopsy of the prostate would be recommended. Under the best of circumstances, cancer diagnoses will be missed and many will undergo unnecessary biopsy, etc. The focus of this paper was the cost-utility of screening with the PSA test. The cost differences between screening and not screening was compared. A Markov analysis and a Monte Carlo simulation were utilized to provide a basis for conclusion. The primary finding is that the costs associated with screening are much higher than those of not screening. Additional findings are that the specificity of the PSA test, the first year treatment costs, and the utility/outcomes of screening have an impact on the cost-utility of screening. The conclusion, however, is that the improvements in these factors would have to be significant to impact the case for screening. Until longitudinal studies are completed, it is not cost-effective to screen for prostate cancer.

Between 1992 and 2009, 29 US states adopted laws mandating that health insurance plans cover screenings for prostate cancer. Because prostate cancer screenings are used almost exclusively by men over 50, these mandates raise the cost of insuring older men relative to other groups. This paper uses a triple-difference empirical strategy to take advantage of this quasi-random natural experiment in raising the cost of employing older workers. Using IPUMS data from the March Supplement of the Current Population Survey, this paper finds that the increased cost of insuring older workers results in them receiving 3% lower hourly wages, being 2.2% less likely to be employed, and being 2% less likely to have employer-sponsored health insurance.
