La fabrication d'un médicament nécessite un niveau élevé de soin et d'attention défini par des réglementations intitulées bonnes pratiques de fabrication (BPF). Ce référentiel officiel a pour but de garantir que tout médicament produit se conforme à des exigences de sécurité d'emploi, de pureté et d'identité. En France, tout établissement pharmaceutique doit se soumettre aux exigences des BPF intégrées au code de la santé publique. De plus, toute activité d'une entreprise fait partie d'un processus ben défini. Les recommandations énoncées dans la norme ISO 9000 implique que chaque processus doit être décrit simpliment sous forme de diagramme. Il est donc fortement conseillé pour une entreprise d'identifier et de regrouper l'ensemble de ses processus dans un document unique tel qu'un Manuel Qualité. Ce dernier peut porter sur la totalité des activités de l'entreprise ou sur une partie de celle-ci. Dans le cas du présent manuel mis en place sur un site de production de vaccins, seuls les processus qualité sont cartographiés. le manuel Qualité est aujourd'hui un outil de communication et d'information offrant à chaque collaborateur une vision générale et synthétique du système de gestion de la qualité, de la politique qualité, de l'organisation interne de l'établissement, ainsi que des processus associés à la qualité des produits.

Suite à différent scandales sanitaires ayant touché le monde du médicament et leurs conséquences pour la santé des patients, une prise de conscience, au niveau mondial, des autorités de santé sur le besoin de faire évoluer et harmoniser les réglementations internationales et nationales a eu lieu. Cette prise de conscience, par les pays membre de l'ICH, a permis la création d'un ensemble de guidelines encadrant toutes les phases du cycle de vie du médicament, en mettant à disposition des outils et systèmes de gestion de la qualité permettant d'augmenter le niveau de qualité des productions pharmaceutiques. C'est notamment sur la base de ces guidelines que les pays membres de l'ICH ont créé des guides de bonnes pratiques de fabrications (BPF pour la France) pour encadrer et inspecter, sur la base d'un référentiel, les pratiques de production des médicaments. Il est donc impératif pour une industrie pharmaceutique d'être conforme à ces critères et de maitriser toutes les étapes de sa production pharmaceutique. L'un des principaux outil qualité permettant cette maitrise est la rédaction d'instructions de production. L'objectif de cette thèse sera de présenter quelles sont précisément les exigences requises par les BPF pour l'élaboration d'une instruction de production et également les outils qualités permettant de répondre à ces exigences. C'est sur la base de ces éléments que sera créé un mode d'emploi opérationnel permettant d'établir rapidement si une instruction de production est conforme aux BPF ou non. Cette thèse présentera enfin une démonstration de l'utilisation de ce mode d'emploi dans un cas pratique réel rencontré dans l'industrie pharmaceutique.
