Brief, individual interventions that provide either risk and normative feedback or normative feedback alone have been shown to particularly efficacious with high-risk college drinkers. The relative efficacy of the two types of feedback has not been determined, however. The current study compares the efficacy of these two interventions with a sample of high-risk college drinkers. It also provides preliminary data on purported mechanisms of change: risk- and norm-perception. College students (n = 1197) were screened for alcohol consumption and alcohol-related problems. The top quartile of this sample (n = 299), in terms of alcohol use severity, was randomly assigned to one of three treatment conditions: risk and normative feedback, normative feedback alone or assessment only. All treatment conditions included a baseline assessment session and the feedback conditions included an in-person feedback session. One hundred and fifteen participants successfully completed their assigned condition. Thirty-eight participants (33%) completed an assessment six months later. Overall, findings did not replicate the existing literature supporting the efficacy of these feedback interventions. Neither feedback group showed appreciable, unqualified reductions in alcohol consumption or alcohol-related problems at follow-up. Primary predictors of alcohol use and alcohol-related problems at follow-up were the baseline measures of these variables. Furthermore, the relationships between baseline and follow-up measures were often qualified by an interaction effect, with individuals who consumed more alcohol or experienced more alcohol-related problems, relative to other participants, being more likely to have a negative response to treatment. Finally, there were no group differences on purported mechanisms of change. Findings may be explained by methodological and sample differences between this study and other, related studies, as well as by social and cognitive mechanisms.

Alcohol is the sixth leading risk factor for disability and premature death all over the world, and one of the leading causes of premature mortality in western societies; it is a leading risk factor for death in young and middle-age males. Heavy drinking accounts for about two thirds of the burden of disease attributable to alcohol. In the early 1980s, screening and brief interventions (SBI) in primary health care settings were proposed as effective strategies to identify risky drinkers and to help them reduce their drinking. Since then, a growing body of evidence, including several meta-analysis and Cochrane reviews, has shown the efficacy and effectiveness of SBI in primary health settings. However, demonstrating the effectiveness of SBI has not been insufficient to facilitate its general implementation in the routines of primary health care physicians, and in fact the dissemination of SBI has proven to be a difficult business. Qualitative and quantitative research has identified most of the facilitators and barriers for its implementation, and publicly funded research has been earmarked to address the dissemination problems worldwide. Some examples are the World Health Organization Phase III and Phase IV studies on the identification and management of alcohol-related problems in primary care, EU funded projects (PHEPA, AMPHORA, ODHIN, BISTAIRS), the UK SIPS trials and the SBIRT developments sponsored by the Substance Abuse & Mental Health Services Administration (SAMHSA) in the USA. The efficacy and effectiveness of SBI in primary health is now well established, but there are still some questions that remain unsolved: which practitioners should deliver them; what length should they be; is there a need for booster sessions; is there added value of a motivational approach? These questions, together with other relevant aspects of SBI, need ongoing research. In recent years, SBIs have been tested in settings other than primary health care, including hospitals, accident and emergency rooms, criminal justice, colleges and universities, social services and pharmacies. In some of those areas, the evidence is scarce (for example, pharmacies) while in others it is very promising (for example, students and hospitals). New technologies have also offered the possibility of online tools, and, in the last few years, different digital-based applications have been tested successfully as new ways to deliver effective SBIs to larger amounts of people. Brief interventions have also spread to drugs other than alcohol. This book aims to be an update of the state-of-the art of brief advice. It is a compilation of articles published by some of the most relevant researchers in the field in Frontiers in Psychiatry between 2014 and 2016.

Rationale: Excessive alcohol consumption among college students leads to serious consequences (e.g. fatal and non-fatal injuries, alcohol poisoning, blackouts, academic failures, violence, rape, assault, unintended pregnancy, sexually transmitted diseases, property damage, and diminished job prospects). Brief alcohol interventions delivered by health care providers have a proven efficacy record, but the economic benefits among college students have not been compared to costs. Objective: Benefit-cost analysis of an ongoing NIH-funded randomized controlled trial of brief alcohol intervention (The CHIPS Project) for at-risk college drinkers at four US and one Canadian university. Methodology: Staff time, facilities, and material costs of screening and brief intervention are compared to reductions in costs of hospital care, emergency response, urgent care treatment and transportation to detoxification centers in the 6 months after randomization. Bootstrap analysis is conducted to form a benefit-cost ratio and confidence interval. Results: 12,900 students entering health services centers at the five participating sites were screened for at-risk alcohol consumption. 4,512 (35%) students screened positive for at-risk drinking, 2,090 completed a face-to-face baseline assessment interview and 986 were randomly assigned to the control or experimental arm of the study. The cost of screening and brief intervention was $106,900, or $217 per study patient. Screening costs accounted for 41% ($44,242) of the total cost with an average of 13.1 screenings per intervention subject enrolled. 24-month follow-up is ongoing, but 6-month results are reported here. At 6-months post-randomization, the hospital, emergency department, and urgent care costs of the intervention subjects was $614 per subject, compared to $491 per subject for controls. The benefit-cost ratio is negative. Conclusions: The CHIPS brief alcohol college intervention did not result in medical care utilization savings. Societal costs and benefits are left for further investigation. This is the first benefit-cost analysis of brief alcohol intervention in a college health care setting.

Background: Nearly half of all undergraduate college students are enrolled in community colleges; yet, the majority of college student health-related research is focused on four-year college students. Alcohol use is one such area where community college research is limited. Research has demonstrated that the use of Protective Behavioral Strategies (PBS) is associated with decreased alcohol use and alcohol-related negative consequences. This dissertation examined short-term changes in the use of PBS, as well as changes in alcohol use and alcohol-related negative consequences as a result of a brief in-class intervention. Methods: Participants were recruited from select courses at a community college. Study participants completed a pen and paper administration of several psychometrically-validated survey instruments that contain items specific to alcohol use, PBS use, and alcohol-related negative consequences. To assess the effects of the intervention, ANCOVA and MANCOVA analyses were conducted. A series of four regression analyses were conducted to test each of three hypothesized mediation models. Results: Two hundred six participants were enrolled in the study and randomly assigned by classroom to an intervention (N = 105) or control (N = 101) group; 179 (87%) participants completed the follow-up survey approximately 3 months later. Analysis of covariance tests did not yield significant differences with respect to overall PBS use, typical number drinks per drinking day, number of heavy drinking days or reported alcohol-related negative consequences between the intervention and control groups at follow-up. The mediation hypotheses were not supported. Conclusions: Participation in the brief intervention did not result in statistically significant changes in alcohol consumption and alcohol-related negative consequences at the time of follow-up. Further in-class brief intervention research needs to be conducted in community colleges to determine the best practices for this setting.
