Multiple dysfunctions take place in the B cell compartment during HIV-1 infection, comprising depletion of resting memory B cells carrying serological memory to vaccines and previously met pathogens. In addition, population of B cells characterized by the expression of exhaustion markers are enlarged during HIV-1 infection. Antibodies with the capacity to neutralize a broad range of HIV-1 isolates can be detected only in a minority of infected patients, after a year or more from acute infection. An open question is whether the inability of producing neutralizing HIV-1 antibodies is somehow linked to the B cell immunopathology observed in patients. In this research topic we invited scientists to summarize the current state of knowledge on regulation and development of B cells and antibody responses during HIV-1 infection; fifteen contributions were received comprising both reviews and original articles. The articles are related to B cell dysfunctions identified in HIV-1 infected individuals, production of different types of antibodies (neutralizing versus non neutralizing, and of different isotypes) in vivo during HIV-1 infection and the biological factors which may impact on this process, clinical potential and applications of anti-HIV antibodies and how to achieve neutralizing antibody responses to HIV-1 epitopes upon vaccination. The topic has gathered articles on front-line research undertaken in the field of B cells and antibodies in HIV-1 infection. It is our hope that the collection of articles presented in this book may be useful for new and experienced scholars in the field and add a piece to the complex puzzle of knowledge needed for the development of an HIV-1 vaccine.

This book considers mainly etiology, pathogenesis, and pathophysiology of aortic aneurysms (AA) and aneurysm rupture and addresses anyone engaged in treatment and prevention of AA. Multiple factors are implicated in AA pathogenesis, and are outlined here in detail by a team of specialist researchers. Initial pathological events in AA involve recruitment and infiltration of leukocytes into the aortic adventitia and media, which are associated with the production of inflammatory cytokines, chemokine, and reactive oxygen species. AA development is characterized by elastin fragmentation. As the aorta dilates due to loss of elastin and attenuation of the media, the arterial wall thickens as a result of remodeling. Collagen synthesis increases during the early stages of aneurysm formation, suggesting a repair process, but resulting in a less distensible vessel. Proteases identified in excess in AA and other aortic diseases include matrix metalloproteinases (MMPs), cathepsins, chymase and others. The elucidation of these issues will identify new targets for prophylactic and therapeutic intervention.

The clinical framework is designed to be "user friendly" so that even the most inexperienced clinician should be able to arrive at the correct diagnosis. The neurological complications of HIV disease have considerably changed in the last decade. Not only have more opportunistic conditions been described but other complications arising from antiretroviral drugs such as lipodystrophy and accelerated atherosclerosis have become problematic. Moreover, the pattern of complications has altered. Diseases such as cerebral toxoplasmosis, AIDS dementia complex and primary central nervous system lymphoma are now becoming uncommon to rare while peripheral neuropathy is becoming increasingly common. In line with these changes, there have been very significant advances in the therapy of HIV disease in general and in neurological complications in particular. Patients with AIDS dementia complex now have a variety of antiretroviral and adjuvant drugs available to them in contrast to the situation only five years ago where therapy was confined to two antiretroviral drugs at best.
