Introduction : Les structures spécialisées en soins palliatifs se sont développées ces dernières années. L'aide aux aidants est devenue un enjeu de santé publique. L'objectif de cette étude est de réaliser un protocole de recherche permettant d'évaluer le bénéfice pour la qualité de vie de l'aidant principal d'un patient souffrant d'un cancer en soins palliatifs de disposer d'un suivi pluridisciplinaire hospitalier compétent en soins palliatifs (HAD) par rapport aux soins de ville. La question de recherche est : existe-t-il un bénéfice de plus de 10 points pour la qualité de vie, évaluée par l'échelle CarGOQoL, de l'aidant principal d'un patient en soins palliatifs pris en charge à domicile par une équipe pluridisciplinaire compétente en soins palliatifs (HAD) par rapport aux soins de ville délivrés en Isère ? Matériel et méthode : Il s'agit d'une étude épidémiologique, observationnelle, de cohorte, multicentrique, comparative, avec recueil unique prospectif des données par autoquestionnaire. Cette étude inclura les aidants de patients en soins palliatifs suivis par l'HAD de Grenoble et ceux suivis en « soins de ville » inclus par les oncologues du CHUGA. Le nombre de sujets nécessaires est de 31 dans chaque cohorte. Les scores de qualité de vie de l'échelle CarGOQoL seront comparés entre les deux cohortes. Conclusion : Ce travail a permis l'élaboration d'un protocole de recherche pour comparer la qualité de vie des aidants de patients souffrant d'un cancer en soins palliatifs entre prise en charge en HAD et soins de ville.

L'hospitalisation à domicile semble être une structure qui répond non seulement aux besoins du patient en soins palliatifs et de son entourage souhaitant être pris en charge à domicile, mais aussi à ceux de la société. Il apparait important de s'intéresser aux proches aidants qui participent activement à la permanence des soins au domicile. L'objectif principal de cette étude est de connaître les représentations des proches aidants de patients en soins palliatifs avancés sur l'HAD. Les objectifs secondaires sont de définir le rôle de l'aidant dans la prise en charge du patient en HAD, et de proposer des pistes de réflexions sur la prévention de leurs fragilités. Une étude qualitative par entretien semi-dirigés a été réalisée auprès de dix proches aidants de patients en soins palliatifs ayant bénéficié d'une prise en charge avec l'HAD Nantes et régions entre trois et six mois avant les entretiens. Les résultats, exploités par la méthode de théorisation ancrée, révèlent que les établissements d'HAD répondent à de nombreuses attentes des aidants que sont la disponibilité physique des soignants, leur organisation, leur soutien moral et la qualité du suivi médical dans la prise en charge. L'entourage peut idéaliser la prise en charge à domicile qui apporte de nombreux avantages dont une part de liberté (entretien du lien social et familial, conservation des habitudes à la maison etc...). L'HAD s'intègre au parcours de soins palliatifs. Chaque aidant s'investit à sa façon et devient « soignant profane ». Néanmoins, face à la réalité de la maladie qui évolue, l'aidant peut se sentir enfermé dans des contraintes que sont souvent l'absence d'alternative au moment de la mise en place du dispositif, la perte d'intimité et de sociabilité, la nécessité d'une présence permanente. Des sentiments légitimes et très présents persistent : la peur de la souffrance, de la mort et l'angoisse de la solitude la nuit notamment. Les perspectives aux vues des résultats ne sont pas innovantes mais insistent sur l'importance de privilégier une démarche palliative incluant tous les professionnels de santé, des spécialistes au médecin traitant. Il s'agirait également de continuer à prévenir les fragilités des aidants, via une prise en charge psychologique personnalisée, mais aussi grâce au développement de séjours de répit et de programmes d'éducation thérapeutique ou en revendiquant une reconnaissance sociale adaptée de l'aidant.

Beaucoup de personnes souhaiteraient passer leurs derniers instants chez eux. Cela nécessite l'investissement de leurs proches, particulièrement l'aidant familial principal dont le rôle est essentiel mais entrainant des conséquences sur sa santé. L'hospitalisation à domicile (HAD) est une alternative à l'hospitalisation conventionnelle. L'objectif principal de cette étude était de détenniner la variation de la santé des aidants familiaux de patients en HAD pour soins palliatifs. L'objectif secondaire était de rechercher un lien entre la variation de la santé des aidants et les composantes objectives et subjectives de leur fardeau. Entre décembre 2011 et août 2013, les aidants de patients admis en soins pallüitifs dans les HAD de l' APARD participaient à un entretien initial puis répondaient à un questionnaire post-HAD afin d'évaluer les éventuels problèmes de santé survenus pendant l'HAD. Le critère de jugement principal était la survenue d'un événement psychique et/ou physique. Les critères de jugement secondaires étaient le fardeau objectif (participation aux soins) et le fardeau subjectif (vécu de son rôle). Panni les 20 aidants analysés, 75% (IC95%[56-94]) rapportaient au moins un événement physique et 30% (IC95%[14,1-55,9)) au moins un événement psychique. La variation de la santé psychique était corrélée au fardeau subjectif (p=0,002 ; t= 0,66 IC95%[0,31 ;0,85]). Cette étude confirme une variation non négligeable de l'état de santé des aidants familiaux de patients en soins palliatifs même dans le cadre d'une HAD. Un travail sur l'évaluation systématique de leur santé et sur l'utilisation du fardeau subjectif pour la prise en charge constituerait une piste intéressante

La plupart des aidants ne se considèrent pas comme soignants mais pensent plutôt appartenir à l’équipe qui entoure le patient. Ils souhaitent en revanche que leur expertise d’aidant soit reconnue. Cette période est reconnue difficile mais n’est pas vécue comme un fardeau, plutôt comme un accomplissement. Être à l’écoute des aidants permet d’améliorer la qualité des soins auprès du patient. Un contrat de partenariat pourrait être créé avec les aidants dans le but de les inclure au projet de soin tout en leur permettant de garder leur place de proche
