Objectif Le mode d'accouchement du singleton à terme en présentation podalique est toujours débattu. L'objectif de notre étude était de comparer la prise en charge per-partum d'une présentation du siège par rapport à une présentation céphalique lors d'une tentative d'accouchement par voie basse. Méthodes Il s'agissait d'une étude rétrospective descriptive comparative monocentrique menée de 2014 à 2017. Nous avons analysé le rythme cardiaque fœtal (RCF) pendant le travail et la phase d'expulsion, la vitesse de dilatation, la durée des différentes phases du travail, le mode d'issue de l'accouchement et l'issue néonatale pour des présentations podaliques et céphaliques. Résultats Deux cent trente-neuf patientes ont été incluses dont 106 (44%) dans le groupe présentation podalique. L'utilisation d'ocytocine était plus fréquente dans le groupe présentation podalique (63,2% versus 48,1%, p = 0,020). La vitesse moyenne de dilatation était plus lente dans le groupe siège que dans le groupe présentation céphalique (1,9 cm/h vs 2,8 cm/h ; p=0,005). Il y avait plus de RCF à haut risque d'acidose dans le groupe présentation podalique (37,2% vs 19,1%, p = 0,001) et la répartition des types de Melchior était comparable dans les deux groupes. Conclusion La gestion per-partum d'un fœtus en présentation podalique est différente de celle d'un fœtus en présentation céphalique. Elle doit être connue et anticipée pour une gestion optimale en salle de naissance.

Introduction : l'accouchement du siège peut être associé à un risque supérieur de morbi-mortalité néonatale. La version par manœuvre externe (VME) est une alternative pour convertir une présentation du siège (PS) en présentation céphalique (PC). La littérature indique une augmentation significative du taux de césarienne après VME réussie (VME+) versus PC spontanée (PCS). Elle compare aussi les issues d'accouchement des PS aux PCS et des PCS aux VME+. Aucune, à notre connaissance, compare les issues maternelles et néonatales des accouchements des VME+ aux PS. L'objectif est de comparer le taux de césarienne entre les deux cohortes ainsi que les caractéristiques du travail et de l'accouchement. Matériel et méthode : étude de cohorte rétrospective multicentrique du 01.01.07 au 30.09.19 qui compare les patientes ayant accouché en PC après VME+ à celles ayant accouché en PS dans 3 maternités publiques des Alpes-Maritimes (France). Appariées sur l'âge maternel, la parité, la date et le lieu d'accouchement, toutes ont accouché à partir de 37 SA d'un singleton vivant. Au total, 258 patientes ont été incluses : 271 dans la cohorte « Sièges » et 87 dans celle « VME+ ».
