Objectif de l'étude : comparer deux stratégies de dépistage systématique du diabète gestationnel, sur deux périodes dans le service de gynécologie-obstétrique du CHU de Grenoble, la première période correspondant à des patientes dépistées par test de O'Sullivan/HGPO 100g, la deuxième période à un groupe de patientes dépistées par HGPO 75g. Matériel et méthodes : étude descriptive rétrospective, monocentrique. 350 patientes ont été incluses dans le groupe 1 (accouchements du 1 Janvier au 16 février 2010, dépistage par test de O'Sullivan/HGPO 100g). 350 patientes ont été incluses dans le groupe 2 (accouchements du 1 Janvier au 17 février 2011, dépistage par HGPO 75g). Ont été exclues secondairement les patientes présentant un diabète antérieur à la grossesse ainsi que les non-dépistées. Résultats : la prévalence du diabète gestationnel a été proche dans les deux groupes, 17.3% en 2010 et 16.16% en 2011. La prise en charge s'est améliorée en 2011, avec un meilleur suivi des glycémies capillaires ainsi qu'un suivi plus fréquent par un diabétologue. L'instauration d'une insulinothérapie, les complications en cours de grossesse ou d'accouchement, ont été comparables dans les 2 groupes. Un quart des patientes n'avaient aucun facteur de risque. Conclusion : contrairement aux résultats de l'étude HAPO, la prévalence du diabète gestationnel dans notre centre n'a pas été plus élevée en 2011. Les deux groupes étaient comparables, en terme de complications en cours de grossesse et à l'accouchement.

Objectif : comparer deux stratégies de dépistage du diabète gestationnel: Ia stratégie en deux temps, test de O’SuIIivan puis HGPO à 100 g et celle en un temps, l'HGPO à 75 g avec les critères diagnostiques de l'étude HAPO. MATERIEL ET METHODE : nous avons mené un essai clinique prospectif randomisé qui a débuté Ie 15 mars 2010 et s’est achevé le 31 novembre 2010 dans Ie service d’obstétrique du CHRU de Brest. Toutes les grossesses mono-foetales répondant aux critéres d'inclusion (suivi de grossesse au CHRU de Brest, âge de Ia patiente ≥18 ans, non diabétique connue) ont été randomisées en deux groupes : d’un côté HGPO à 75 g et d’un autre côté O’SuIIivan puis HGPO à 100 g. Lorsqu’un diabète gestationnel a été diagnostiqué, Ia même prise en charge a été mise en place dans les deux groupes, en collaboration avec Ie service d'endocrinologie du CHRU. RESULTATS. L'anaIyse des résultats a mis en évidence : une diminution du terme de dépistage du diabéte gestationnel et une diminution du délai de prise en charge dans Ie groupe HGPO à 75 g. On note aussi une tendance à Ia diminution du taux de macrosomie, de déclenchements, d'accouchements instrumentaux, de césariennes dans Ie groupe HGPO à 75 g ces résultats ne sont statistiquement pas significatifs du fait d’un petit échantillon. Concernant Ia tolérance par rapport aux tests de dépistage, I’HGPO à 75 g est aussi bien tolérée que Ie O’SuIlivan.CONCLUSION. Les résultats de ce travail sont en accord avec les dernières recommandations du CNGOF parues en décembre 2010, à savoir de dépister Ie diabète gestationnel par I’HGPO à 75 g.

Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being?
