Across the world, welfare states are under challenge—or were never developed extensively in the first place—while non-state actors increasingly provide public goods and basic welfare. In many parts of the Middle East and South Asia, sectarian organizations and political parties supply basic services to ordinary people more extensively and effectively than governments. In sub-Saharan Africa, families struggle to pay hospital fees, and nongovernmental organizations (NGOs) launch welfare programs as states cut subsidies and social programs. Likewise, in parts of Latin America, international and domestic NGOs and, increasingly, private firms are key suppliers of social welfare in both urban and rural communities. Even in the United States, where the welfare state is far more developed, secular NGOs and faith-based organizations are critical components of social safety nets. Despite official entitlements to public welfare, citizens in Russia face increasing out-of-pocket expenses as they are effectively compelled to seek social services through the private market In The Politics of Non-State Social Welfare, a multidisciplinary group of contributors use survey data analysis, spatial analysis, in-depth interviews, and ethnographic and archival research to explore the fundamental transformation of the relationship between states and citizens. The book highlights the political consequences of the non-state provision of social welfare, including the ramifications for equitable and sustainable access to social services, accountability for citizens, and state capacity. The authors do not assume that non-state providers will surpass the performance of weak, inefficient, or sometimes corrupt states but instead offer a systematic analysis of a wide spectrum of non-state actors in a variety of contexts around the world, including sectarian political parties, faith-based organizations, community-based organizations, family networks, informal brokers, and private firms.

Evidence indicates that actions within four main themes (early child development fair employment and decent work social protection and the living environment) are likely to have the greatest impact on the social determinants of health and health inequities. A systematic search and analysis of recommendations and policy guidelines from intergovernmental organizations and international bodies identified practical policy options for action on social determinants within these four themes. Policy options focused on early childhood education and care; child poverty; investment strategies for an inclusive economy; active labour market programmes; working conditions; social cash transfers; affordable housing; and planning and regulatory mechanisms to improve air quality and mitigate climate change. Applying combinations of these policy options alongside effective governance for health equity should enable WHO European Region Member States to reduce health inequities and synergize efforts to achieve the United Nations Sustainable Development Goals.

The purpose of the research summarized in this paper is to provide policy-relevant knowledge on the governance of rural services in Guatemala and thus to contribute to improving the provision of services that are essential for agricultural and rural development. Almost 10 years ago, the Guatemalan government decided to strengthen decentralization and community participation to improve the quality of public services, as well as access to these services for the poor, especially in rural areas. Based on quantitative and qualitative primary data, we examine how services are actually provided today and how community preferences and participation affect service provision in rural Guatemala. Our main finding is that the provision of formally decentralized services by local governments is incomplete. As a result, many rural communities continue to lack access to services, and some of them engage in supplying these services themselves. However, communities do not consider themselves to be more effective at service provision and would therefore prefer to be served by the government. Moreover, we find that community participation in the planning and evaluation of services has a positive impact on the responsiveness of the local governments service provision. However, the effectiveness of community participation varies greatly among the examined cases, with several participatory governance bodies not functioning properly due to low education levels, poverty, and weak civil-society organization.

Marja-Liisa Swantz has spent a lifetime conducting participatory action research in Tanzania, and In Search of Living Knowledge encapsulates her reactions. She started her career in 1952 in Tanganyika as an instructor to the first generation of women teachers at Ashira Teachers Training College, situated on the slopes of Mount Kilimanjaro. In the first years of Tanzanias independence from Britain, she devoted five years (1965-1970) to participant research in a coastal Zaramo village near the capital city of Dar es Salaam. The research culminated in her book, Ritual and Symbol in Transitional Tanzanian Society, and a doctorate in Anthropology of Religion, which she received from the Swedish University of Uppsala in 1970. The author further developed the Participatory Approach to research while serving as a Senior Research Fellow at the University of Dar es Salaam from 1972 to 1975. After becoming a lecturer at the University of Helsinki she continued to develop Participatory Action Research with Tanzanian and Finnish doctoral candidates in a project in Bagamoyo, Tanzania, known as Jipemoyo. She continued to apply the participatory approach in research projects as Director of the Institute of Development Research at the University of Helsinki, where she taught anthropology, and as a Senior Researcher at the World Institute for Development Economics Research Institute in Helsinki in the 1980s. Since retirement, the author has continued her research, writing, and participation in development projects in Tanzania, including projects in Mtwara and Lindi from 1992 to 1998, and for 12 years while involved in a Local Government Cooperation project between Hartola in Finland and Iramba in Tanzania.

Silent Violence engages the harsh reality of malaria and its effects on marginalized communities in Tanzania. Vinay R. Kamat presents an ethnographic analysis of the shifting global discourses and practices surrounding malaria control and their impact on the people of Tanzania, especially mothers of children sickened by malaria. Malaria control, according to Kamat, has become increasingly medicalized, a trend that overemphasizes biomedical and pharmaceutical interventions while neglecting the social, political, and economic conditions he maintains are central to Africa’s malaria problem. Kamat offers recent findings on global health governance, neoliberal economic and health policies, and their impact on local communities. Seeking to link wider social, economic, and political forces to local experiences of sickness and suffering, Kamat analyzes the lived experiences and practices of people most seriously affected by malaria—infants and children. The persistence of childhood malaria is a form of structural violence, he contends, and the resultant social suffering in poor communities is closely tied to social inequalities. Silent Violence illustrates the evolving nature of local responses to the global discourse on malaria control. It advocates for the close study of disease treatment in poor communities as an integral component of global health funding. This ethnography combines a decade of fieldwork with critical review and a rare anthropological perspective on the limitations of the bureaucratic, technological, institutional, medical, and political practices that currently determine malaria interventions in Africa.
