Introduction : Les violences sexuelles ont un impact sur la santé globale des femmes. Il existe notamment un moins bon suivi gynécologique chez les patientes aux antécédents de violence sexuelle. La façon dont les médecins généralistes vont examiner ces femmes peut influencer leur recours au soin .Méthode : Une étude qualitative a été menée à l'aide d'entretiens semi-dirigés auprès de 10 médecins généralistes en Normandie et en Savoie, entre janvier et mai 2023. L'analyse a été réalisée dans une approche qualitative phénoménologique et par théorisation ancrée avec triangulation des données. Résultats : La saturation des données a été atteinte après 10 entretiens. Le médecin généraliste assure la prise en charge globale de ces patientes. Il adapte son examen et est attentif au bien-être de la patiente. Il veille à créer une relation de confiance avec sa patiente. Il reste cependant mal à l'aise face à ce sujet avec des difficultés de dépistage systématique des violences et de coordination avec le réseau professionnel. Des formations existent au sujet des violences auprès des femmes et la question de comment les intégrer au sein de la formation initiale ou continue parait indispensable devant la forte prévalence des violences. Conclusion : Il existe un changement de pratique avec des médecins plus sensibilisés mais qui se sentent encore en difficulté pour assurer un bon dépistage et une bonne prise en charge des femmes aux antécédents de violence sexuelle.

Introduction : La consultation gynécologique, enjeu de santé publique, permet la prévention des grossesses indésirables et le dépistage des cancers. Pourtant des inégalités de santé ont été observées pour les populations migrantes primo-arrivantes avec peu d'études leur donnant la parole. L'objectif était de déterminer l'expérience de ces dernières, leurs représentations et attentes de cette consultation afin de comprendre ces inégalités. Méthodologie : Cette étude qualitative a été conduite entre juin 2020 et janvier 2021, auprès de femmes migrantes primo-arrivantes en France depuis moins de 5 ans, recrutées à Vannes et à Rennes. Les données ont été analysées de façon thématique, après codage des verbatim. Résultats : Quatorze entretiens individuels ont été réalisés avec traducteur pour les patientes non francophones. La santé reproductive semblait être une priorité et une opportunité pour débuter le suivi gynécologique, avec une contraception peu sollicitée et parfois jugée négativement. Elles sollicitent peu le dépistage des cancers gynécologiques par défaut d'information, contrairement aux MST. La migration provoque, pour la plupart, un changement des pratiques antérieures générant pour certaines une opportunité de suivi gynécologique ou à l'inverse de l'incompréhension. L'examen gynécologique parfois appréhendé en cas d'antécédents de violences sexuelles semble majoritairement bien vécu, contrairement aux difficultés de communication et à la barrière de la langue. Les patientes ont un désir de participation active au sein de la relation médecin malade et un désir d'information, souvent relayé par le réseau associatif et amical. Conclusion : Les participantes ont une diversité de représentations de la santé sexuelle et reproductive et un manque d'information pouvant influencer le suivi. Le médecin généraliste , les groupes de paroles entre femmes avec présence d'un traducteur, ainsi que les actions ciblées jouent un rôle central pour repérer, orienter et informer les femmes migrantes et pourraient diminuer les inégalités en santé observées.
