La demencia frontotemporal (DFT) representa un grupo de enfermedades neurodegenerativas muy heterogéneo desde el punto de vista clínico, neuropatológico y genético. El espectro clínico engloba tres subtipos: demencia frontotemporal variante conductual (DFT-vc), demencia semántica (DS) y afasia progresiva no fluente (APNF). Existe también superposición con la enfermedad de motoneurona (EMN), así como algunos síndromes parkinsonianos (parálisis supranuclear progresiva, PSP, y degeneración corticobasal, DCB). A nivel histopatológico la degeneración lobar frontotemporal (DLFT) se caracteriza por una atrofia predominante de los lóbulos frontales y temporales asociada a acúmulos de proteínas anómalas en las neuronas o la glía. Se han descrito tres grupos principales: DLFT con patología tau (DLFT-Tau), DLFT asociada a la proteína fijadora de ADN TAR-43 (DLFT-TDP) y DLFT asociada a la proteína de fusión en sarcoma (DLFT-FUS). En los últimos años se han identificado varios genes causantes de la DFLT: MAPT (proteína asociada a microtúbulos), PGRN (progranulina), VCP (valosin-containing protein), CHMP2B (charged multivesicular body protein 2B), FUS (fused in sarcoma protein) y TARDBP (TAR DNA binding protein).
