Contexte : le vieillissement de la population française s'est accru ces 20 dernières années et les chiffres continueront d'augmenter dans les années à venir. Le bilan de médication chez la personne âgée a un rôle d'aide pour ces patients : optimisation de leur traitement, apport d'informations. Objectif : mener les bilans de médication à l'officine sur un échantillon de patients, observer les caractéristiques de cette population et évaluer leur satisfaction vis-à-vis de ce bilan. Méthodes : il s'agit d'une étude prospective chez des patients de plus de 75 ans, ayant au moins 5 médicaments sur leur ordonnance à l'aide d'un questionnaire et d'un entretien avec le patient. Des données du patient ont été collectées, pendant 6 mois, ses traitements et leur gestion ainsi que la satisfaction du patient envers ce bilan. Résultats : parmi les 483 patients inclus, les pathologies cardiovasculaires sont les plus répertoriées avec une part importante de dépression (20%). Les trois types de vieillissement sont répartis équitablement. Il a été observé un nombre important de médicaments prescrits par patient (8,4). Le taux d'automédication observé était important (53%). Les patients ont été satisfaits du bilan de médication (note de 9/10) et ont exprimé un fort besoin d'informations sur leur prise en charge médicamenteuse. Conclusion : notre étude suggère que le bilan de médication est un outil apprécié par les patients, qui sera une des nouvelles missions du pharmacien au sein du système de santé.

La population vieillissante sujette aux maladies chroniques et polymédiquées entraine une augmentation des accidents iatrogènes responsable d'hospitalisations faisant de ce fait un enjeu de santé publique. C'est dans ce contexte qu'en 2009 la loi HPST énonce dans ses textes la mise en place des bilans partagé de médication (BPM) et confie cette nouvelle mission aux pharmaciens d'officine. S'agissant d'entretiens structurés avec le patient, le BPM permet d'optimiser la prise en charge médicamenteuse du patient et a pour objectif, entre autres, de réduire l'iatrogénie, améliorer l'adhésion du patient, réduire le gaspillage médicamenteux et améliorer la collaboration médecin/pharmacien. L'objectif de cette thèse était d'analyser les BPM réalisés dans le cadre d'une expérimentation conduits par des étudiants en 6ème année officine dans les facultés de pharmacie de Lille et Lyon. Cette dernière s'est déroulée de janvier à juin 2018 et a permis d'identifier différentes données sociodémographiques, pharmaceutique et relever certaines difficultés à l'instauration de ces BPM dans le quotidien des officines. De futures expérimentation de ce genre pourraient à terme permettre d'identifier plus de leviers et de freins à la mise en place de cet outil voire d'étudier l'impact pharmacoéconomique du BPM.
