L'Hospitalisation A Domicile (HAD) s'est développée ces dernières années. L'organisation du circuit des produits de santé est une composante essentielle de la prise en charge des patients. Le partage de l'information est une problématique dans la gestion à distance des intervenants de santé. La transmission des prescriptions effectuées au domicile est représentative de cette problématique. L'objectif de ce travail est d'évaluer l'apport des nouvelles technologies de l'information et de la communication (NTIC) pour contribuer à sécuriser le circuit des produits de santé en HAD. Pour cela une étude de faisabilité de l'utilisation de téléphones mobiles disposant de fonctions de communication évoluées (Smartphones) pour transmettre en temps réel les prescriptions a été mise en place au sein de l'HAD de La Rochelle. Ce système de communication a été évalué afin de déterminer la qualité de la transmission et l'apprentissage du système. Après 24 semaines les résultats montrent que ce système est fonctionnel. Les prescriptions médicales numérisées au chevet du patient peuvent être transmises via un réseau sans fil au pharmacien chargé de la dispensation. Par ailleurs, on observe que les professionnels de santé de l'HAD utilisent le système au delà du cadre strict du circuit des produits de santé pour échanger des informations utiles à une prise en charge optimisée du patient. Une utilisation au quotidien des NTIC pour sécuriser le circuit des produits de santé en HAD apparaît donc envisageable si les questions soulevées lors de cette expérience (fiabilité des transferts, qualité des images, sécurisation, confidentialité et cryptage) trouvent des réponses satisfaisantes du point de vue technique et réglementaire.

Les objectifs de qualité de la prise en charge médicamenteuse suggèrent l'informatisation et l'automatisation comme outils pour son amélioration. À partir de lignes directrices publiées, une auto-évaluation des processus entourant l'implantation et l'utilisation d'armoires automatisées dans les unités de soins a été menée au centre hospitalier universitaire Sainte-Justine de Montréal. Cette analyse de conformité des pratiques a permis d'orienter les actions, tant sur le plan technologique qu'organisationnel, dans le but d'atteindre une sécurisation optimale de la prise en charge médicamenteuse. En parallèle, plusieurs études mettent en évidence l'apparition d'erreurs médicamenteuses générées par l'implantation d'une nouvelle technologie. Il est donc nécessaire d'identifier les non-conformités à des lignes directrices publiées afin de connaître, prévenir et limiter les risques associés. Les outils disponibles pour les évaluer sont actuellement insuffisants. A cet effet, établir un cadre optimal d'utilisation parait indispensable pour chacune des solutions d'informatisation et d'automatisation du circuit du médicament.
