La rétention des deuxièmes molaires permanentes, bien que rare, devient un phénomène de plus en plus fréquent. Elle peut être liée à des obstacles mécaniques ou des défauts intrinsèques du mécanisme d’éruption (PFE: Primary Failure of Eruption décrit par Proffit et Vig en 1981). De brefs rappels de terminologie permettent d’utiliser le vocabulaire approprié. Ce travail reprend en détail les connaissances actuelles des phénomènes d’éruption, ainsi que les différentes voies de recherche, la voie RANK/RANKL/OPG au niveau du follicule dentaire est particulièrement intéressante. Puis, ce mémoire reprend les données des principales études réalisées sur les défauts d’éruption depuis les années 70. Ensuite, tous les éléments diagnostic sont repris de manière détaillée afin de cerner précisément l’anomalie, ce qui conditionnera les choix thérapeutiques. Notamment, des mutations du gène PTH1R ont été mises en évidence dans les cas de PFE héréditaire. Un arbre décisionnel permettra de distinguer: retard d’éruption, impaction (Mecanical Failure of Eruption), et rétention primaire ou secondaire associée à une PFE type I, II ou III (?) "Dental fluorosis refers to changes in the appearance of tooth enamel that are caused by long-term ingestion of fluoride during the time teeth are forming. Studies conducted in the 1930s showed that the severity of tooth decay was lower and dental fluorosis was higher in areas with more fluoride in the drinking water. In response to these findings, community water fluoridation programs were developed to add fluoride to drinking water to reach an optimal level for preventing tooth decay, while limiting the chance of developing dental fluorosis. By the 1980s, studies in selected U.S. communities reported an increase in dental fluorosis, paralleling the expansion of water fluoridation and the increased availability of other sources of ingested fluoride, such as fluoride toothpaste (if swallowed) and fluoride supplements. This report describes the prevalence of dental fluorosis in the United States and changes in the prevalence and severity of dental fluorosis among adolescents between 1986-1987 and 1999-2004. In their contribution to the first edition of this Handbook, entitled "The Teeth," LEHNER and PLENK (1936) discussed the tissues constituting the "perio dontium" rather briefly. In contrast to the detailed paragraphs dealing with, for example, enamel and dentine, the section (about 40 pages and 20 illustra tions, mostly drawings) devoted to periodontal tissues failed to provide a factual review and summary of the contemporary knowledge and latest developments in research on the various components of the periodontium. Instead, much of the text was an attempt to arrive at conclusions from often purely semantic speculations, playing the various schools of thought against each other, provid ing arguments in favor of the authors' views and arguments for the feasibility and probability of accepting or rejecting the often diverse opinions, while the reader was referred to the already existing literature for factual details. Since 1936, however, factual details ofthe structural biology of the periodon tal tissues, i. e. their development, structure, function, and physiology, have been greatly extended and have been internationally accepted. With much less opin ionated belief to cope with, this knowledge has formed the solid foundation upon which diagnosis, prevention, and treatment in the fields of clinical perio dontology, modem orthodontics, and re- and transplantation procedures of teeth have been built.
