On May 25, 1979, American Airlines Flight 191, a McDonnell-Douglas DC-10-10 aircraft, on its way from Chicago to Los Angeles, crashed just after take-off near Chicago-O'Hare International Airport, Illinois. During the take off the left engine and pylon assembly and about 3 ft of the leading edge of the left wing separated from the aircraft and fell to the runway. Flight 191 crashed killing two hundred and seventy one persons on board and two persons on the ground. The accident remains the deadliest airliner accident to occur on United States soil.

On August 27, 2006, Comair Flight 5191, a Bombardier CL-600-2B19, crashed during takeoff from the wrong runway of Blue Grass Airport, Lexington, Kentucky, killing 49 of the 50 people aboard. From the beginning everything went wrong. First the captain and first officer boarded the wrong airplane, only after starting the auxiliary power unit they found out they were in the wrong aircraft. Taxiing to the takeoff position the captain and first officer were so engaged in a private conversation that they did not realize they took the wrong runway. Flight 587 was a scheduled passenger flight to Santo Domingo, Dominican Republic, with a crew of 9 and 251 passengers aboard the airplane. Shortly after take-off the airplane lost its tail, the engines subsequently separated in flight and the airplane crashed into a residential area of Belle Harbor, New York. All 260 people aboard the airplane and 5 people on the ground were killed, and the airplane was destroyed by impact forces and a post crash fire.
