La prevalence du retard de parole et/ou de langage chez les jumeaux est superieure à celle des singletons. En effet, le contexte d'acquisition du langage est fortement influence par la situation triadique entre la mere et les jumeaux. Dans le cadre d'une reeducation orthophonique que nous avons pu observer, nous avons souhaite identifier les situations d'interaction qui pouvaient faciliter la prise de parole de l'enfant. Pour cela, des enregistrements videos ont ete effectues au domicile de deux familles de jumeaux âges de 5 ans 7 mois et 6 ans 2 mois et ne presentant qu'un retard de parole et de langage sans troubles associes. Quatre situations ont pu etre filmees : un repas, une lecture d'album, un moment de jeu et une seance d'orthophonie. Cette etude a suggere que les enfants parlaient plus en terme de nombre d'enonces produits et de longueur moyenne d'enonces en situation de lecture d'album mais que la qualite de leurs enonces par rapport à leurs troubles etait meilleure en seance de reeducation. Cela a donc permis de mettre en evidence l'importance d'une prise en charge orthophonique associee à une guidance parentale personnalisee afin de permettre une remediation plus rapide des troubles.

L'intervention orthophonique faisant participer les parents permet de redonner à ceux-ci la place qui leur revient auprès de l'enfant et de replacer le retard de langage dans sa véritable perspective : celle d'un trouble de la communication qui prend sa source, et par conséquent sa solution, dans les interactions verbales entre cet enfant et ses parents. En partant de ce principe et à travers un questionnaire destiné aux parents d'enfants souffrant d'un retard de langage, nous avons tenté d'analyser les attitudes communicatives impliquées dans le développement harmonieux du langage oral. Nous nous sommes intéressée plus particulièrement à la période 0-30 mois pour mettre en évidence la qualité des interactions verbales et non verbales. Selon les résultats, nous avons regroupé les comportements parentaux sous différentes rubriques jusqu'à isoler finalement certaines attitudes constituant des facteurs de risque de retard de langage. L'objectif de notre étude a été donc double : 1) Identifier et répertorier les facteurs de risque importants dans les attitudes communicatives des parents ; 2) Insister sur la prévention pendant la petite enfance à partir des données recueillies. Ainsi, les résultats de notre travail ont fourni un argument de plus en faveur de l'intervention active des parents dans la prise en charge orthophonique et de l'action ciblée de l'orthophoniste pour freiner leurs comportements à risque et induire un changement qui pourra éviter les erreurs avec les enfants à venir.

Le retard de parole et de langage est un trouble assez fréquent en maternelle. Il peut être bénin et se résorber rapidement mais il peut aussi apparaître sous des traits beaucoup plus sévères, entravant alors la communication entre l'enfant et son entourage. A l'école, ces troubles peuvent fragiliser les apprentissages et se répercuter sur d'autres domaines (relations, comportement, estime de soi...) Ainsi, ce mémoire traite des impacts du retard de parole et de langage sur les interactions entre l'enfant et son enseignant en grande section de maternelle. Nous émettons l'hypothèse d'une influence négative sur la prise de parole, sur la pertinence des réponses de l'enfant ainsi qu'une influence sur les stratégies d'étayage de l'enseignant et sur la réussite de l'échange. La mise en place d'une activité de langage dans 7 petits groupes, autour d'un livre sans texte, a permis la cotation des comportements de 14 enfants (7 ayant un retard, et 7 sans trouble) sur une grille d'observation et la comparaison entre ces deux groupes d'enfants en interaction avec leur enseignant. Nous constatons que le retard n'entrave pas la prise de parole de l'enfant mais qu'il peut le rendre moins informatif. Ce trouble entraîne manifestement des stratégies d'étayage plus importantes qu'avec l'enfant tout-venant, se caractérisant principalement par des reformulations et des questions ouvertes. La réussite de l'échange est assurée en grande partie grâce aux interventions de l'enseignant pour soutenir le discours de l'enfant. Malgré leur retard, les enfants s'intègrent à la vie de la classe, et particulièrement aux activités de langage.

Cette étude aborde l'opposition retard de langage versus dysphasie et la validité de cette distinction chez quatre enfants âgés de 4 ans 7 mois à 5 ans 10 mois. Ces enfants présentaient un retard sévère d'acquisition du langage oral lors de l'étude de V. Aubin en 2005. Nous avons tenté de préciser la nature de la pathologie langagière de chaque sujet. Pour cela nous nous sommes appuyés sur des corpus de langage semi-induits fournis par des enregistrements audio et vidéo réalisés lors d'une conversation entre l'enfant et l'expérimentateur orthophoniste. La L.M.P.V. et l' I.D.L. corrigé ont été calculés et une analyse « grammaticale» des types de mots produits selon Bassano a été effectuée. Nous avons également procédé à une analyse qualitative et nous avons étudié l'évolution langagière des sujets en comparant nos données à celles que V. Aubin avait obtenues. Nos observations montrent que 2 sujets arborent un retard global de développement avec une déficience intellectuelle auquel s'associe un retard de langage encore très important; un troisième sujet présente un retard de langage que nous ne qualifierons pas de « simple» étant donné la très grande pauvreté du milieu socio économique et culturel dans lequel évolue l'enfant; enfin le dernier sujet pose encore question: son langage a bien progressé mais certains éléments (dyspraxie éventuelle, contexte médical lourd) nous font douter d'un simple retard sachant que nous n'avons pu mettre en évidence ici une déviance caractéristique.

Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training.
