Les personnes âgées sont confrontées à une polymédication grandissante, une augmentation du risque d'iatrogénie médicamenteuse et des problèmes de tolérance. Ces médicaments deviennent potentiellement inappropriés pour elles à cause d'une balance bénéfice/risque défavorable. L'objectif de notre travail est d'analyser l'identification des médicaments potentiellement inappropriés (MPI) par les étudiants de 6e année officine dans le cadre des bilans partagés de médication (BPM) réalisés pendant leur stage de pratique professionnelle. La liste STOPP/START dans sa 2ème version a été utilisée pour identifier les MPI. Les critères d'inclusion étaient : âge ≥ 65ans et ≥ 5 molécules prescrites. 356 BPM ont été inclus. Les patients ont en moyenne 1,2 critères STOPP, 0,8 critère START et 2 critères STOPP et START. Parmi les 356 BPM, 741 (46,3 %) médicaments sont considérés comme potentiels MPI du fait de données incomplètes, 655 (40,9 %) n'ont pas été vus par les étudiants et 205 (12,8 %) l'ont été. Les catégories pour lesquelles on retrouve le plus de MPI avérés sont pour les critères STOPP : la neuropsychiatrie (35,7%), la hors catégorie (22,0%) et la cardiovasculaire (8,2%) et pour les critères START : la respiration (51,9%), la locomotion (17,2%) et la cardiologie (15,4%). Des leviers d'actions existent pour améliorer la détection des MPI : former à la déprescription, des stages en gériatrie, des unités d'enseignements consacrées aux outils spécifiques des MPI. L'iatrogénie médicamenteuse n'étant pas l'affaire que du pharmacien, il serait intéressant de programmer des temps d'échanges avec les étudiants en médecine et infirmiers afin de communiquer ensemble sur cette thématique.

Le bilan partagé de médication est un acte pharmaceutique autorisé en France depuis mai 2018. Il permet au pharmacien de réaliser, avec des patients âgés de plus de 65 ans, une revue exhaustive de l'ensemble de leurs traitements et de détecter les médicaments potentiellement inappropriés pour proposer des interventions pharmaceutiques à leurs médecins généralistes. Ce bilan est également l'occasion d'évaluer l'observance et les difficultés liées aux thérapeutiques que rencontrent les patients dans leur vie quotidienne. Très peu d'études ont été réalisées pour évaluer l'efficacité des bilans partagés de médication ou pour comprendre les difficultés rencontrées lors de leur mise en place. Ce travail a ainsi porté sur une évaluation qualitative de la mise en place de ce nouveau service, permettant d'en faire ressortir les différents leviers et freins ; mais aussi sur une étude quantitative ayant pour objectif d'analyser les médicaments détectés comme potentiellement inappropriés, les problèmes liés et les interventions pharmaceutiques qui en découlent. Les principales motivations concernaient une forte envie de participer au parcours de soin des patients et d'améliorer les relations interprofessionnelles. Le principal frein à la mise en place des bilans était un manque de temps. 60% des bilans analysés ont donné lieu à au moins une intervention pharmaceutique. 15% des médicaments analysés étaient considérés comme potentiellement inappropriés. La principale classe à risque était les inhibiteurs de la pompe à protons. Bien que le bilan partagé de médication soit un outil efficace pour améliorer la prise en charge médicamenteuse des patients, d'importants freins à leur déploiement sont rencontrés par les pharmaciens d'officine. Il conviendrait donc de repenser cet acte à la lumière de ces freins, avec l'ensemble des parties prenantes, pour une plus large application sur le territoire.
