Odd grains of uric acid produced in the body each twenty-four hours, the liver destroys I8 grains, the kidneys 22, the muscles 3 or 4, and the remainder, about 6 grains passes out of the body in the urine. If foods are habitually taken which contain a considerable quantity of uric acid, it is stored up in the tissues, the system being unable to destroy it. The result is hardening of the arteries, enlargement and hardening of the liver, premature Old age, the formation of calculi in the kidneys or bladder, neuralgias, nervous exhaustion and probably a greater variety of symptoms and dis orders than those resulting from any other single cause. Excerpt from Albany Medical Annals, 1905, Vol. 26: Journal of the Alumni Association of the Albany Medical College One member of the French school of the past century, Louis, famous on account of the introduction of a new method of scien tific observation, is of particular interest to us on account of his influence upon American medicine. His new method, now one of the commonest, was that of reaching conclusions by numerical analysis. As the result of this system he was the first to demon strate the futility of indiscriminate bleeding and blistering; but is probably best known for his great work on fevers, especially typhoid fever. At the time (1829) this last appeared the Paris school was in great favor with American students; Osler6 has col lected definite records of thirty-seven who afterwards attained more or less prominence at home. To several of these (gerhard, James Jackson, Jr., George C. Shattuck and Alfred Stille), stirnu lated by the influence of Louis's teaching, is due the credit of distinguishing between typhoid and typhus fevers and of definitely establishing the pathology of each - one of the most brilliant pieces of work based on close observation and correct deduction of which we have record in clinical medicine. To complete this sketch we must take a glance at therapy, the ultimate object, it must be admitted, of all investigations in medi cine, though curiously enough the advance in therapeutics has never been commensurate with the general progress. Until a hundred years ago therapy varied according to the prevailing school or system, always, however, with venesection as its sheet anchor and obscured by polypharmacy. This is not surprising, however, when we consider. That treatment was limited almost entirely to drugs - the old hygienic teaching of the early Greek physicians was now forgotten - and that the use of these drugs was almost entirely the result of chance, tradition or even of supersti tion. The manner in which many therapeutic measures arose is indicated in the following summary of Oliver Wendell Holmes' which refers to a few specifics which our art did not discover, and is hardly needed to apply. Excerpt from Albany Medical Annals, Vol. 40: Journal of the Alumni Association of the Albany Medical College; July, 1919 In recent work in the army camps they found only about one in ten of positive culture showed virulence so that not all carriers are dangerous. If you take a culture in a community when diphtheria is present, you will find that the ratio is considerably larger. Five or six per cent are positive, especially in camps and institutions. It is not always the finding of the morphological germ of diphtheria which gives us such great concern. Then another problem which we have to contend with is reluctancy in giving antitoxin early. Schick has shown us why we should give antitoxin early. He has shown us that antitoxin has 'no value after the toxin of the diphtheria has entered into the cell tissues. The only way by which we can expect the antitoxin to do any good is to catch the toxin while it is still in the blood cur rent. Vve must give antitoxin in the early period otherwise much damage will be produced. I do not know how it is with you, but we have often heard it said that you must continue to give anti toxin so long as the symptoms are bad. If you give a single dose of antitoxin and give a large enough dose at one time, there is no use of giving any more at all. The only time you should give more is when you are in doubt as to whether you have given enough in the first dose. Give sufficient dose at the earliest pos sible moment. The question comes up as to the method of administration of antitoxin. One of the things I want to take up today is to settle in our minds what we expect to do with antitoxin. Schick has worked out a very excellent scheme for dosage. He states that in 90 per cent of cases we should give 100 units for each kilogram of weight.
