L'obésité est une maladie chronique aux origines multifactorielles dont la prévalence est en constante augmentation. En France, en 2012, 15 % des français étaient atteints d'obésité. Actuellement, le seul traitement efficace de l'obésité morbide (IMC > 35) est la chirurgie bariatrique qui est une intervention lourde regroupant un ensemble de techniques complexes. Le pharmacien est de plus en plus confronté à ces patients en amont et en aval de leur prise en charge. L'objectif était d'évaluer les connaissances des pharmaciens d'officine sur l'accompagnement du patient obèse avant et après chirurgie, et identifier les besoins d'accompagnement des patients obèses. Notre enquête a montré que 94 % des pharmaciens d'officine comptaient parmi leur clientèle un ou plusieurs patients obèses, et qu'il semblait important qu'ils acquièrent les connaissances et compétences nécessaires pour pouvoir conseiller, délivrer des médicaments et orienter. Les patients lors des entretiens menés, sont demandeurs d'interactions avec les pharmaciens pour être accompagnés mais constatent quelque fois des insuffisances dans leurs connaissances. Des outils pratiques de formation tels que des diaporamas disponibles en e-learning, une foire aux questions et un dépliant ont été réalisés.

L'obésité fait partie des maladies chroniques. Il s'agit d'une pathologie complexe qui demande une prise en charge multidisciplinaire : rééquilibrage alimentaire, activité physique adaptée et suivi psychologique. Dans ce cadre, l'Éducation Thérapeutique du Patient (ETP) est une approche particulièrement pertinente. A ce titre, il a toute sa place dans l'accompagnement du patient en situation d'obésité, cette mission d'ETP lui ayant été attribuée par la loi HPST (« Hôpital, Patients, Santé et Territoires ») de 2009. Toutefois, le manque de temps et de rémunération peut rendre la pratique d'ETP difficile à mettre en œuvre à l'officine. En réponse à cette problématique, ce travail propose un protocole d'intervention brève applicable au comptoir. America's health care system has become too complex and costly to continue business as usual. Best Care at Lower Cost explains that inefficiencies, an overwhelming amount of data, and other economic and quality barriers hinder progress in improving health and threaten the nation's economic stability and global competitiveness. According to this report, the knowledge and tools exist to put the health system on the right course to achieve continuous improvement and better quality care at a lower cost. The costs of the system's current inefficiency underscore the urgent need for a systemwide transformation. About 30 percent of health spending in 2009-roughly $750 billion-was wasted on unnecessary services, excessive administrative costs, fraud, and other problems. Moreover, inefficiencies cause needless suffering. By one estimate, roughly 75,000 deaths might have been averted in 2005 if every state had delivered care at the quality level of the best performing state. This report states that the way health care providers currently train, practice, and learn new information cannot keep pace with the flood of research discoveries and technological advances.
