Early childhood caries (ECC) is a problematic disease that has been on the rise in young pre-school age children within the last decade. Children who have untreated dental disease early in life, are at increased risk of having poor oral health throughout their lifetimes. Approximately 70% of dental disease is found in only 20% of the nation's high-risk children. Professional organizations and governing bodies have formed several initiatives in order to help lower the prevalence of ECC in children. One such initiative, early preventive dental visits, i.e. dental home, has proven to be successful; yet, the evidence is limited in documenting its effectiveness. First Dental Home (FDH) is the state of Texas Medicaid initiative to improve access to care for children. FDH was initiated to improve oral healthcare for children aged 6 months to 35 months of age by providing simple, consistent messages regarding proper oral healthcare to the parents/caregivers of the children. Despite the large fiscal budget allocated towards the success of FDH, no studies regarding the program's effectiveness have been published to date. This study aimed to evaluate the effectiveness of the FDH by comparing the knowledge, practice and opinions of participating vs. non-participating parents regarding their young children. A 29-question survey was given to mostly low-income parents who visited qualifying Medicaid clinics in North Texas (Dallas/Fort Worth) and South Texas (Harlingen). A total of 165 parents completed the survey. Several significant results emerged between the knowledge and practices responses of the parents sampled. On the knowledge section, FDH parents responded correctly more often than the non-FDH parents when asked about the recommended amount of toothpaste recommended for toddlers (p=0.023). Oral health disparities are profound worldwide, and they affect the quality of life of individuals of all age groups. Disparities in oral health are seen in racial and ethnic minorities, at different socioeconomic levels and due to differences in environment and cultural factors. Several determinants of oral health have been identified at the population, community, family and individual levels. These determinants represent a complex interplay of the social, biological, cultural and economic factors that in turn affect the oral health behaviors, environmental exposures, health care utilization. To date, biological factors related to oral diseases have received much attention in oral health research; whilst social and cultural determinants have just started to receive recognition for their role in oral disease development and progression. This research highlights that interventions designed to reduce disparities should adopt a multi-level approach in order to identify the modifiable mechanisms and target all determinants of oral health disparities. All rights not granted by the above license are retained by the author. Abstract: Early childhood caries is a condition of rapidly progressing rampant dental caries in infants and young children. It is considered as a major public health problem affecting pre-school children and it is the most common dental disease which affects 28% of children within two to five years of age. ECC not only induces pain and discomfort, but can also affect communication, nutrition status, learning abilities, speech and quality of life, which may progress into adulthood and pose a heavy burden on the healthcare system in long-term. In Hong Kong, over 50% of children were affected by dental caries in 2001 but over 70% of children had never been to a dentist for a regular check up at age five. Thus, much of the tooth decay was remained undetected and untreated. Dental caries can be preventable and achievable. Collaboration between families, early care and health care professionals is required to promote effective oral health care. Numerous studies have found that educational programs and workshops are effective in promoting oral health and can provide children a lifelong opportunity to be free from preventable oral disease. In order to promote oral health to achieve the mission of the Department of Health in Hong Kong- at least 65% of 5 years old children are free from caries by the year 2020, an evidence-based guideline for a community-based programme in oral health education was developed in the proposed setting after a critical appraisal of the reviewed evidence. The comprehensive intervention plan, including communication plan with stakeholders, training of staffs and pilot testing will be carried out to facilitate the implementation of the innovation. The oral health programme will be evaluated for its effectiveness in achieving the patient outcomes, health providers' outcomes and system outcomes in the proposed settings.
