Introduction : Les pneumopathies aigues communautaires restent graves et fréquentes chez l’enfant. Les recommandations anglo-saxonnes ou françaises les plus récentes montrent l’absence de diagnostic clinique clair alors même que le reste de la prise en charge y est particulièrement bien décrite. Ainsi on comprend tout l’intérêt pour la médecine générale d’une étude sur cette étape clef qu’est le diagnostic clinique. Matériel et méthode : Etude rétrospective d’une série de 283 enfants dont la conclusion diagnostique était une pneumopathie aiguë communautaire étayée par un foyer de condensation alvéolaire sur une radiographie du thorax. Le recrutement a eu lieu du 1er janvier 2009 au 31 décembre 2010 aux urgences du CHRB. Le recueil de données et l’analyse se sont spécifiquement concentrés sur les signes cliniques le jour de l’examen et sur ceux de l’histoire de la maladie qu’ils soient généraux, pulmonaires et surtout extra- pulmonaires. Résultats : Sur les 283 cas de l’étude, les pneumopathies touchent surtout l’enfant de moins de 3 ans (2/3 des cas ; p=0,03) de manière épidémique sans différence selon le sexe (p= 0,66). Il conduit le plus souvent à une prise en charge ambulatoire (p=0,15). Il se dégage un socle commun associant une évolution typiquement bruyante (2/3 des consultations avant J3 ; p= 0,005) et rapide (33,6% d’aggravation dès J1 ; p=0,049). On y regroupe aussi une fièvre élevée (81,4% ; p=0,01) et une toux (94,1% ; p=0,01) et/ou une rhinite (75,2% ; p=0 ,02). Une altération de l’état générale est rare (13,1%) à la différence de difficultés à l’alimentation (32,5% ; p=0,02). Le tableau clinique pulmonaire est constitué d’un tableau de retentissement pulmonaire (41,5% ; p=0,01) et de signes auscultatoires notamment en foyer (24% ; p=0,03). On note un tableau « atypique » digestif (48,4% des cas associé avec des signes pulmonaires (p= 0,01) et 33,2% sans (p=0,04)). Le tableau neurologique est peu fréquent (7,7% ; p=0,001) ainsi que les douleurs thoraciques (6,7%) ou les signes ORL (angines : 7,4% et otites 4,9%). Conclusion : Ces résultats détaillent le tableau clinique de cette pathologie et permettent une vision globale de celle-ci. L’amélioration des connaissances sur cette étape clef de la prise en charge d’une pathologie principalement ambulatoire qui reste grave et fréquente chez l’enfant trouve tout son intérêt pour la médecine générale
